Home and Community Based Services (HCS)
Wellness Healthcare Solutions provide HCS services in 25 counties. The HCS program is a Medicaid wavier program that provides services to individuals with mental retardation who either live with their family, in their own home, in a foster/companion care setting or in a residence with no more than four individuals who receive HCS services. Wellness Healthcare Solutions provide all specialized services under HCS.
- Case Management service delivery is coordinating the therapeutic function for working with those in need of particular kinds of human services. We incorporate excellence in the delivery of the essential elements of case management: client in-take, assessment of need, service planning, service coordination and linking, and the monitoring and continuous evaluation of the client, of service delivery, and of available resources.
- Foster/Companion Care is provided by a foster care provider that the client resides in a home setting. The foster care provider is responsible for ensuring that the consumer’s needs are met daily. Wellness Healthcare Solutions pays Foster/Companion Care 100% of the daily rate. When inquiring with any company it is important to ask for 100% daily rate.
- Supported Home Living provides focused skills training in community settings to enhance a person’s independence.
- Respite Care provides short term temporary or intermittent direct care of a child or adult, either in-home or our of home. It is designed to relieve the caregiver of stress resulting from continuous care of a dependent person or persons.
- Employment Services Program is a job placement and job-coaching program for adults. Consumers learn job readiness skills and then put those skills to use in finding the community job that is best suited to them. The goal of the program is to assist consumers in being as independent as possible in living and working in the community.
- Referrals to Structured day activity programs that provide services designed to assist in the acquisition, retention, and/or improvement in adaptive skills necessary for community living.
- Partnerships with Vocational Training provides training to assist consumers to become employed or to increase skills in daily living.
- Nursing Services provide services consist of performing health care procedures and monitoring the individual’s health conditions.
