Community Living Assistance and Support Services (CLASS) Case Management

The CLASS program provided by Wellness Healthcare Solutions Inc. provides home and community based case management services to consumers with related conditions as cost effective alternatives to placements outside the home. The CLASS program is funded by Medicaid wavier which allows the Texas Department of Human Services to provide services that are not available under the regular Medicaid program.

The goals for CLASS are to:

• Create potential for personal growth, power, and increased self-reliance;


• Integrated people with related conditions into the community;


• Provide flexible services which reflect the changing individual needs of people with related conditions; and


• Prevent institutionalization.

Our CLASS Case Management Program is based on the premise that, with education and personalized and individualized assistance, consumers can acquire the knowledge and develop the skills necessary to manage their lives and comply with the treatment plan. On an ongoing basis, Case Managers work face to face and via telephone to develop self-management through basic education, referral, crisis intervention, re-assessment and monitoring. The intensity and type of intervention is customized based on the consumer’s individual needs and abilities.

Case Managers will be available 24 hours per day to facilitate emergencies or crisis. Our staffing model includes 24 hour availability which is dispatched and monitored through our daily call system. Our auto-assignment capacity is managed through the automated call center.

Wellness Healthcare Solutions provides Case Management Services for the CLASS program in Dallas, Longview and Wichita Falls with offices in each area.
